New Clients


Please review the following information prior to your first meeting with one of our providers:  

Office Location: Our practice is located in Chicago’s Lincoln Park neighborhood, at 1901 N. Clybourn Avenue, Suites 302 and 304 (waiting area for clients is in suite 302). The office building entrance is located on Kenmore Ave., just east of Clybourn (enter from the parking lot behind the MooseJaw store). Please enter the building and then use the electronic directory located inside the building to search for your provider’s name, or call 773-697-8839 to reach your provider directly. Once inside the office building, please take the elevator to the third floor, and turn right when you exit the elevator. Our waiting room for all clients is located in suite 302.

Parking and Public Transit for 1901 N Clybourn Location: There is typically ample metered street parking on Clybourn Ave, and additional zoned street parking is available on the surrounding residential streets. The Armitage ‘L’ stop (brown and purple express lines) is located 2 blocks from our practice, and the North/Clybourn ‘L’ stop (red line) is located approximately 3-4 blocks from our new location. Please do not park in the attached lot, as we do not currently have assigned client parking available in this lot and your car may be ticketed or towed if you park in a spot designated for another business.

Intake paperwork: Once you have set up an intake meeting, your clinician will email you with the relevant intake paperwork. Please complete these forms in advance and bring them with you to your first session, or notify your provider if you prefer to have these forms printed for you to complete during your intake session. 

Payment and Insurance: Payment for services is always due at the time of service, and may be made by cash, check, or credit card (including HSA cards, when applicable). All clients are required to keep a current credit card on file, which will be charged following each billable service, unless an alternate form of payment is provided at the time of service. 

Our practice is out-of-network for insurance. At your request, we will be happy to provide you with statements that include all information needed to seek out-of-network reimbursement from your insurance carrier. Out-of-network reimbursement rates vary based insurance carrier, plan and service provided. 

Cancellation Policy: To avoid a last-minute cancellation fee, please notify us at least 24 hours prior to your appointment to cancel or reschedule.
